Job Overview: As a Transitional Support Officer, you will play a key role in guiding young people through the transition from child to adult services. This process, which typically begins around the age of 14 and continues until around 25 years old, involves significant changes such as leaving school, finding employment, forming new relationships, and securing independent living arrangements. Your role will be to offer practical support, guidance, and person-centred planning to ensure a smooth and empowering transition for each individual.

Key Responsibilities:

  • Transition Planning: Work closely with young people and their families to develop and implement transition plans that address educational, employment, housing, and social needs. Use person-centred planning techniques to build independence and support each individual’s goals.

  • Independent Living Support: Assist young people in finding suitable housing and adapting to independent living. Provide support with day-to-day tasks such as paying bills, cooking, laundry, and home security, tailored to their specific needs.

  • Job Support: Help individuals explore employment opportunities, complete job applications, prepare for interviews, and navigate the world of work. Offer guidance to enhance confidence, social skills, and financial independence.

  • Personal Relationships: Support young people in building and maintaining personal relationships and integrating into the community. Provide discreet guidance while respecting their privacy and fostering a positive social life.

  • Financial Management: Assist with understanding and managing finances, budgeting, and money management to help individuals handle their new responsibilities as they transition to adulthood.

Requirements:

  • Experience: Experience working with young people with special needs or disabilities is preferred but not essential. We offer full training to equip you with the skills needed for the role.

  • Skills: Strong interpersonal skills, empathy, and the ability to build positive relationships with young people and their families. Patience and a positive attitude are crucial.

  • Flexibility: Ability to work flexibly to meet the needs of individuals and families, including potentially variable hours based on client requirements.

  • Training: Full training will be provided to support you in delivering effective transitional support.
